RECOVERY SUPPORT NAVIGATOR TEAM

Monroe County

What We Do

The Monroe County Recovery Support Navigator program is designed on an innovative and flexible peer support model, which allows staff to meet individuals and families where they are located as we collaborate with them to help strengthen their recovery.  The Recovery Support Navigator Team works to help families and individuals combat substance use disorders through a blended model of peer and family support services.  Our supports are judgement free, recovery oriented, trauma informed, hope and empowerment driven, community focused and revolve around what is important to the individuals and families we serve.

PEER ENGAGEMENT

Our Peer Specialists provide emotional support, share insights about the treatment process, and assist people in obtaining services to support their recovery, including substance and opioid use treatment, support groups, peer support and community services.

FAMILY SUPPORT

Our Team provides support for family members and loved ones that are designed to guide, educate, and support families through the substance used disorder treatment and recovery process.  We also assist families with accessing local treatment and others services.

FIT PEER TEAM

Our Peer Specialist provide follow up support and guidance to individuals referred by the Monroe County Forensic Intervention Team, who have recently experienced a mental health and/or addiction crisis.

HOMELESS PARTNERSHIP PROGRAM

In partnership with multiple agencies such as MCOMH, PCHO, and MC Collaborative, Liberty Resources reaches out and engages housing deprived individuals who need assistance with residential needs and support their recoveries around addiction and/or mental health.

SUPPORT GROUPS

We offer support groups to facilitate meaningful connections while supporting and enhancing the recovery process.

FAMILY GUIDANCE & SUPPORT GROUP

The Recovery Support Navigator Team runs the Family Guidance & Support Group both in-person and virtually to support the needs of families as their loved ones go through their journey with addiction and the recovery process.  Support topics include:  what is addiction, the science of addiction, how to be supportive of a loved one in recovery, how to enhance the recovery process, family roles,  self-care & coping skills, provide emotional support, and inspire hope.
