Our Permanent Housing programs provide much-needed housing supports for individuals who are homeless or at risk of homelessness. Our supportive services are tailored to meet the unique needs of each participant. This is established through individual assessment and the collaborative development of an individualized service plan. We support their current needs and offer case management services, supportive counseling—including crisis intervention, ongoing assessment, and independent-living skills training.

Substance Use - OASAS MRT, Upstate Perm Supportive Housing

Substance use disorders often create housing barriers for individuals. Finding and maintaining safe housing is a crucial component in recovery. Our permanent housing programs provide services that are appropriate to the needs of each individual we serve, with the goal of greater independence.

Supportive services are developed to meet the needs of each individual we serve. This is established through individual assessment and the development of an individualized service plan. Program staff, either directly or through linkages, assist tenants with accessing resources that will enhance their ability to secure employment, including educational opportunities, job-readiness skills, vocational training, and employment placement and retention. Apartments are turnkey, and all tenants have the option to take over responsibility of the lease when it is determined that our services are no longer needed.

ELIGIBILITY INFORMATION

The Permanent Supportive Housing program provides services to adults, age 18 or older, with a documented substance use disorder.

Mental Health - OMH Supportive Site

Mental health issues can impact many facets of daily living including access to permanent housing, a crucial component of maintaining an individual’s health. Our Mental Health Supportive Housing programs provide services and financial supports for individuals transitioning from a residential setting and also for individuals who are at risk of homelessness.

  • HOUSING PLACEMENT:  Our Housing Counselors ensure that individuals have the ability to exercise their right to choose where they are going to live, taking into consideration the recipient’s functional skills, the range of affordable housing options available in the area, and the type and extent of services and resources that recipients require to maintain their residence within the community.
  • FINANCIAL SUPPORTS:  The program provides financial assistance to recipients who meet income guidelines. Liberty Resources successfully provides rental stipends, moving costs, security deposits, and utility assistance to more than 80 recipients a year.
  • CASE MANAGEMENT SERVICES:  Housing Counselors offer case management services, supportive counseling—including crisis intervention, ongoing assessment and support planning, and independent-living skills training. We encourage participants to develop natural community supports and to use community resources to strengthen their bond to their neighborhood and encourage integration into community.

ELIGIBILITY INFORMATION

For enrollment, please contact the Onondaga County Mental Health Department’s Adult Single Point of Access (SPOA) team at 315-435-7711 ext 4696 or visit them online.  The county accepts SPOA referrals online.  The SPOA team determines eligibility and priority status according to New York State Office of Mental Health criteria. 

Chronically Homeless - HUD Permanent Supportive Housing

The HUD Permanent Supportive Housing (PSH) program assists persons with substance-use disorders and mental health issues that create barriers to permanent housing. The HUD PSH program is composed of 29 units spread throughout Onondaga County. Each tenant has a private apartment and is responsible for an appropriate portion of the rent every month.

    HIV / AIDS

    Liberty Resources provides permanent housing to individuals and families who are homeless or housing vulnerable through our MESA Commons site, and through our housing retention program. Each program focuses on the individual’s overall needs to help find and maintain safe and affordable housing. Permanent housing is crucial in maintaining the overall health and happiness of those we serve.

    HIV/AIDS Housing Retention Program

    Provides long-term rental assistance/subsidies as financial support for housing, enabling the individuals with HIV/AIDS to maintain stable housing and medical care; acquire skills and stability; and successfully achieve independent living. Peer Specialists function as part of the housing retention services team and assist with client education, supportive counseling, and escorting clients to medical and/or housing retention appointments.

    Community Living

    Liberty Resources operates several Supervised Residential programs, and it provides Supported Community Living to a variety of individuals throughout Onondaga and Madison Counties. Emphasis is on creating an environment that encourages the individuals to experience a full life. All individuals create goals for themselves based on their unique desired outcomes, as expressed during their annual Life Plan meeting. Our Community Living programs prepare people for more-independent living.

    Liberty Resources provides multiple housing options and supports in Syracuse for people with HIV/AIDS, addiction, and mental health issues that prevent permanent housing. Financial Assistance and Housing Retention Services enable individuals/families to maintain stable housing and to successfully achieve independent living. Peer Specialists function as part of the housing retention services team and assist clients.
