Care Coordination is provided through the NYS Health Homes program and is specifically designed to help children, families and adults connect with an array of services to address all of their needs; physical health, behavioral health, education and social supports. Individuals and families will have one point of contact, their Care Manager, who provides guidance and support with enrolling and accessing services to stay healthy, succeed at school and remain involved in their communities.

The Care Coordination services offered by Aurora of CNY provide access to and coordination of healthcare providers and community supports so that individuals can meet their goals of living a productive, healthy and independent life. Coordinating with providers for special issues relating to vision and/or hearing loss needs is a specialty of their care and support.
