Same Day Access designates dedicated times when our team provides walk-in assessment and connection to resources. Appointments are not required for Same Day Access. During your visit, you will undergo a needs assessment and have the opportunity to speak with a Care Coordinator or Clinician about your concerns. This lays the foundation for your care plan, a road map that helps meet the goals that you outline with our team of professionals.
Same Day Access time slots operate on availability and need. Services vary by location. It may take up to four (4) hours to complete your Same Day Access visit.

What happens during Same Day Access?
You will meet with a Care Coordinator or Clinician who will help you complete a Needs Assessment, which asks questions about your current symptoms, your healthcare history, previous treatment plans, your housing status and daily living needs, and more. They will discuss the Needs Assessment with you and outline your next steps. You will also need to complete a New Patient Enrollment form. Additional information may also be requested. You may not be able to see a provider immediately. Please contact 911 if you are experiencing an urgent issue that needs immediate medical attention.

This service helps expediate the process that clients must go through for Liberty Resources to understand their needs. You do not need an appointment to come for Same Day Access services. You may not be able to see a provider immediately.
Who should come for Same Day Access?
Any individual who is five years or older and is looking to receive services at Liberty Resources.
What should I bring to Same Day Access?
If you are coming to Liberty Resources for Same Day Access, please bring the following.
- Insurance Card
- Custody and/or Guardianship Paperwork, if applicable
If you do not have insurance, Liberty Resources will still provide services to you regardless of your ability to pay. We have a Sliding Fee Scale for qualifying individuals. Please email our Benefits Team for more information.
I came for Same Day Access but was unable to be seen. What should I do?
We do our best to provide intake session for each client, but only have a certain amount of timeslots available during our Same Day Access hours. If you are unable to be seen, you can come back on the next day that Same Day Access intakes are available
