STABILITY IN YOUR HOME ENVIRONMENT

Liberty Resources understands that having stable housing is key to maintaining the physical and emotional health of the people we serve. We offer residential services that offer support from people who take the time to understand what our residents need and help them connect to the right resources.

Please note: Liberty Resources does not offer financial assistance for housing or rental assistance or security deposits unless you are enrolled in one of our programs. Please contact your local Department of Social Services for available resources.

More Than Housing

Our residential services teams work with each individual to develop a plan with meaningful goals with an approach that supports hope and healing.  Residents also get access to additional services that support their independence and help them maintain a stable living environment.

          • Connection to programs and services that help individual’s work towards the goal that they set.  This may include: transportation, primary care, care coordination, substance use treatment, mental health counseling, peer support, and more.
          • Supportive counseling including crisis intervention, ongoing assessment, and independent-living skills training to help residents gain independence.
          • Trained staff members help develop individualized service plans, outlining milestones that assist clients in maintaining their recovery and mental health while achieving housing, employment, and educational stability.

Our Housing Programs

HIV & AIDS

DePalmer House

Residential program through the NYS Office of Mental Health.

ABOUT THIS PROGRAM

    • Transitional residential program with the goal of helping resident attain affordable permanent housing
    • Safe living environment in a two-story house with 10 bedrooms, shared living spaces, and program office space for confidential counseling areas.
    • Program is staffed 24/7/365

ELIGIBILITY INFORMATION

    • Individuals 18 years of age or older with a
    • Documented diagnosis of HIV or AIDS
    • Considered unhoused / homeless or have inadquate housing status and fall below the federal poverty line.
    • Residents are requested to attest, in writing, their willingness to participate actively in their service and to abide by program rules.

MESA Commons

Residential program through the NYS Office of Mental Health.

ABOUT THIS PROGRAM

    • Permanent housing program for individuals living with HIV or AIDS compromised of two- and three-bedroom townhouse units with private entry through a community courtyard.
    • Units are equipped with standard amenities including full bathroom, refrigerator, stove, air conditioning, and a washer and dryer.
    • Leases are required on a month-to-month basis.  Rent includes heat, electricity, hot water, and garbage collection.
    • No security deposit is required.

    • Individuals 18 years of age or older
    • Documented diagnosis of HIV or AIDS
    • Considered unhoused / homeless or have inadequate housing status and fall below the federal poverty line.

HIV / AIDS Housing Retention Program

Residential program through the NYS Office of Mental Health.

ABOUT THIS PROGRAM

    • Supports individuals living with HIV / AIDS with stable housing through long-term rental assistance and subsidies
    • Peer Specialist assist with client education, supportive counseling, and escorting clients to medical and/or housing retention appointments

    • Individuals 18 years of age or older
    • Documented diagnosis of HIV or AIDS
    • Considered unhoused / homeless or have inadequate housing status and fall below the federal poverty line.

Mental Health

Apartment Treatment Program

Residential program through the NYS Office of Mental Health.

ABOUT THIS PROGRAM

    • Designed for individuals who have thrived in structured environments while their recovery journey and are now prepared to embrace greater flexibility and independence.
    • Residents must adhere to prescribed medication routine, remain abstinent from harmful substance,  maintain a clean living environment and be proficient in basic cooking skills
    • Our goal is to provide a model for healthy living so residents can move into a more independent setting.
    • Apartments are scattered throughout Madison County
    • Program staff are available around the clock, 365 days a year but not on-site

Madison County Mental Health Department’s Adult Single Point of Access (SPOA) department determines eligibility and priority status.  To be considered for the Apartment Treatment Program, applicants must be 18 years of age or older with a mental health diagnosis such as major depressive disorder, bipolar disorder, or schizophrenia.

Venture House

Venture House exteriorResidential program through the NYS Office of Mental Health.

ABOUT THIS PROGRAM

    • Designed for individuals with serious and persistent mental health diagnosis and/or co-occurring substance use disorders 
    • Residents share responsibility for general housekeeping and daily chores
    • Dinner is cooked and served in a family-style manner while residents are responsible for preparing their own breakfast and lunch with staff support
    • Staffed 24/7/365

ELIGIBILITY INFORMATION

    • Individuals 18 years of age and older who are
    • Documented diagnosed with a DSM-V Axis I disorder—classified as a serious and persistent mental illness. To request an application, call the program direct

Empire State Supportive Housing Initiative (ESSHI)

Residential program through the NYS Office of Mental Health.

ABOUT THIS PROGRAM

    • Designed for families or individuals with serious mental illness
    • Provides stable, supportive housing for vulnerable populations under the Housing First model
    • Tenant holds lease and subsidized payment is made directly to landlord or housing agency

ELIGIBILITY & ENROLLMENT

    This program’s eligbility and enrollment is determined by the Single Point of Access (SPOA) teams in Onondaga and Madison counties.

    • ONONDAGA COUNTY:  Contact the Onondaga County Mental Health Services’ Single Point of Access (SPOA) team at 315-435-7711 ext. 4696 or visit them online.
    • MADISON COUNTY:  Contact the Madison County Mental Health Department’s Adult Single Point of Access (SPOA) team at 315-366-2327 or visit them online.

    Single Point of Access (SPOA) teams in these counties determine eligibility and priority status.  To be considered, applicants must be 18 years of age or older with a mental health diagnosis such as major depressive disorder, bipolar disorder, or schizophrenia.

    HUD Permanent Supportive Housing (PSH)

    Residential program through the US Department of Housing and Urban Development.

    ABOUT THIS PROGRAM

      • Designed for families or individuals with serious mental illness
      • Consists of affordable housing units across Onondaga County 
      • Tenants are responsible for a portion of rent based on their income

    ELIGIBILITY 

      • Must be must be 18 years old and have a documented disability or have a family member with a documented disability
      • Must be chronically unhoused / homeless
      • Must be on Onondaga County’s Continuum of Care Coordinated Entry List

    ENROLLMENT

    To enroll in this program, a referral must be entered in Onondaga County’s Continuum of Care Coordinated Entry List.

    Substance Use

    Maxwell House

    ABOUT THIS PROGRAM

      • Designed to support individuals in recovery who benefit from a structured, stable housing environment with integrated services that promote continued recovery, skill-building, and successful reintegration into the community
      • On-site life skills development as well as recovery supports and planning
      • Staffed 24/7/365

    ELIGIBILITY INFORMATION

      • Adult men 18 years or older with a diagnosis of substance use disorder
      • Meets criteria based on Level of Care determination

    Next Step Supportive Living

    ABOUT THIS PROGRAM

      • Designed for individuals in recovery who are ready for increased independence while still benefiting from supportive services in a community-based residential setting
      • Apartments and housing are scattered throughout Madison County

    ELIGIBILITY INFORMATION

      • Must be 18 years of age or older with a substance use disorder diagnosis.
      • Meets criteria based on Level of Care Determination
      • Have an established support system and recovery plan
      • Individuals across the board must be able to function without 24-hour-a-day on-site supervision.

    Permanent Supportive Housing - Medicaid Re-Design Team

    Madison County Permanent Supportive Housing through NYS Office of Addiction Services & Supports.

    ABOUT THIS PROGRAM

      • Designed to assist individuals striving for substance use recovery with finding permanent housing through rental subsidies and care management services
      • Option to take over responsibility of lease once services are no longer needed
      • Affordable/supportive housing for high-frequency, high-cost Medicaid beneficiaries who are homeless or precariously housed
      • Includes rental subsidies and other occupancy costs for apartments, program supervision, housing, and employment counseling

    ELIGIBILITY INFORMATION

      • Adult 18 years or older enrolled in Medicaid with a documented, primary diagnoses of a substance use disorder
      • Unhoused / homeless or at risk of becoming – this includes persons who are in imminent danger of losing their housing due to sudden change of building ownership OR individuals at immediate risk of losing housing due to:
        • Sudden change in building ownership
        • Eviction notice or pending foreclosure on a rental property
        • Residence condemned for safety reasons
        • Overcrowded living conditions
        • Unsafe environment that hinders recovery, with no alternative housing
        • Sudden, significant loss of income
        • Loss of essential accommodations (e.g., elevator)
        • Damage to the building
        • Discharge from an inpatient facility without a housing plan or necessary support due to substance use disorder
      • Have a qualifying medical history in a 12-month period specific to the 12 months immediately prior to either the date of referral to the MRT housing program or the date of entry into a community residence, supportive living, or other transitional housing program:
        • two medical-based inpatient hospitalizations (not substance use inpatient) OR
        • 5 emergency room visits OR
        • 4 emergency room visits and one inpatient hospitalization

    OASAS Upstate Permanent Supportive Housing

    Madison County Permanent Supportive Housing through the NYS Office of Addiction Services & Supports.

    ABOUT THIS PROGRAM

      • Designed to assist individuals and families striving for substance use recovery with finding permanent housing through rental subsidies, care management services, and employment services
      • Established by NY State
      • Housing which includes rental subsidies, case management, and employment services for individuals/families in recovery in rural and/or small suburban regions of Upstate New York
      • Turnkey apartments with the option to take over responsibility of lease once services are no longer needed

    Transitional Safety Units

    Transitional Safety Units throughout Madison County through NYS Office of Addiction Services & Supports.

    ABOUT THIS PROGRAM

      • Designed for adults with substance use disorders who are experiencing homelessness, cannot currently access permanent housing, and are exiting a residential treatment program or the criminal justice system
      • Provides short-term, supportive housing for 12 to 18 months as the individual pursues permanent housing
      • Includes support for an apartment with the option to take over responsibility of lease once services are no longer needed

    ELIGIBILITY INFORMATION

      • Adults 18 years or older with a documented substance use disorder
      • Unhoused / homeless or at risk of becoming unhoused / homeless
      • Exiting a residential treatment program or the criminal justice system
