People living with disabilities and their families have the same hopes and dreams as everyone else, even though they face different challenges. Every person, regardless of their ability, deserves to be included as full, contributing members of their community, and we’re here to help make that happen. We provide the supports and services that are vital to achieving their goals at every stage of life.
Day Habilitation - now accepting referrals!
Day Habilitation is site-based program that focuses on building skills and involves community outings.
The ratio of staff to individuals at our Day Habilitation programs is kept below 1:3 so that staff can focus on every person’s future goals. Outings provide an opportunity to work on all skills in the community. Liberty Resources strives to assist people in making their own personal connections in their communities through various activities:
- Regular community outings tailored to an individual’s personal interest, such as exercise classes in the community, movies and local festivals, picnics at the local parks, participating in sports games, attending sports events, and visiting pet shelters
- Social and living skills development
- Helping individuals to develop their adult roles in the community and to foster connections in their communities
- Performing community service by deliver meals and cleaning at Habitat for Humanity sites
Our Direct Support Professionals also offer instruction and support with the Activities of Daily Living
- Cooking
- Personal care
- Housekeeping/Cooking/Shopping
- Health and personal safety
Individual Residential Alternative (IRA)
Individual Residential Alternative (IRA) program provides 24-hour care from Direct Support Professionals in a family-style home setting.
Our supervised residential programs provide 24-hour supervision to adults with intellectual disabilities. Each location focuses on each individual’s needs and community inclusion. Our caring Direct Support Professionals are trained in CPR/first aid, medication administration procedures, and crisis intervention techniques. Registered Nurses provide essential oversight and resources at each home—ensuring the highest quality of medical care.
- Health and Wellness: Our staff promote the health and wellness of all individuals through health maintenance and prevention strategies, accurate medication administration, knowledge of general health and safety rules, and first-aid and emergency procedures. Direct-support professionals help individuals complete personal-care activities while ensuring an individual’s privacy and promoting independence.
- Individual Empowerment: We cultivate and promote the ability of the individual to lead a self-determining life by providing the support and information that are necessary to build self-esteem, assertiveness, and independence. Direct-support professionals show respect for individuals by soliciting and honoring choices and by encouraging personal responsibility and independence in all day-to-day events and activities
- Household Management: Direct-support professionals assist the individuals who are responsible for household management (e.g., shopping, meal preparation, laundry, cleaning, and decorating) and transportation, with the goal of maximizing the individual’s skills, abilities, and independence.
- Advocacy: We are knowledgeable about the diverse challenges facing individuals, and we understand our role as an advocate. Our staff understand and communicate individual needs, wants, and choices to his/her family, coworkers, the organization, and the service system. We identify and promote the rights of individuals.
- Facilitation of Services: We provide service plan development, implementation, and review. Our staff know the history, needs, and preferences of the individuals we serve, including characteristics of disability, behavior, cultural background, and medical issues.
Supervised Residential Programs are located throughout Onondaga and Madison counties.
Self-Direction
Self-Direction is a person-centered approach that empowers individuals to choose their own caregivers, programs, services, and supports.
Our Self-Direction Services allow you, along with your parents or advocates, to exercise choices around the types of supports and services you receive and who provides them to you. This is done through an assessment of your needs, your available resources, and the development of an annual budget. Our full spectrum of care within our Developmental Disabilities Division—including our behavioral health, primary care, and full-service pharmacy—is easily accessible, providing access to care that addresses all of your health needs.
Community Habilitation
Community Habilitation is based in the community and empowers individuals by enhancing their abilities, fostering independence, and building relationships.
Our clients and their families can expect training and assistance in areas of need and access to the community for desired activities. Outings provide an opportunity to work on skills in the community. Activities may include visits to local points of interest, attendance at cultural performances, opportunities for exercise and volunteer work, and enjoying time spent with friends. Our Direct Support Professionals can also provide one-on-one instruction and support with the activities of daily living, including:
- Budgeting and shopping
- Maintaining the home or apartment
- Planning for the future
- Personal care
- Health and personal safety
Community Habilitation is available in Onondaga and Madison counties.

Choosing Liberty Resources for your loved one with a developmental disability is a decision you can feel confident about. Our Direct Support Professionals genuinely caring and understand the unique needs of each individual to provide personalized support. Every person we support receives compassionate care in a safe and nurturing environment.
In addition, our agency can provide wrap around the care through its Syracuse Integrated Health Center, which provides outpatient mental health and substance use services as well as primary care.
