STABILITY IN YOUR HOME ENVIRONMENT

Liberty Resources understands that having stable housing is key to maintaining the physical and emotional health of the people we serve. We offer residential services that offer support from people who take the time to understand what our residents need and help them connect to the right resources.

Please note: Liberty Resources does not offer financial assistance for housing or rental assistance or security deposits.  Please contact your local Department of Social Services for available resources.

 Our residential services teams work with each individual to develop a plan with meaningful goals with an approach that supports hope and healing.

  • Connection to programs and services that help individual’s work towards the goal that they set.  This may include: transportation, primary care, care coordination, substance use treatment, mental health counseling, peer support, and more.
  • Supportive counseling including crisis intervention, ongoing assessment, and independent-living skills training to help residents gain independence
  • Trained staff members help develop individualized service plans, outlining milestones that assist clients in maintaining their recovery and mental health while achieving housing, employment, and educational stability

Our Residential Programs

HIV & AIDS

DePalmer House

Residential program through the NYS Office of Mental Health.

ABOUT THIS PROGRAM

    • Transitional residential program with the goal of helping resident attain affordable permanent housing
    • Safe living environment in a two-story house with 10 bedrooms, shared living spaces, and program office space for confidential counseling areas.
    • Program is staffed 24/7/365

ELIGIBILITY INFORMATION

    • Individuals 18 years of age or older with a
    • Documented diagnosis of HIV or AIDS
    • Considered unhoused / homeless or have inadquate housing status and fall below the federal poverty line.
    • Residents are requested to attest, in writing, their willingness to participate actively in their service and to abide by program rules.

Individuals can apply by completing our Residential Application and submitting to residentialservices@liberty-resources.org.  The agency also accepts referrals from hospitals, case management agencies, correctional facilities, drug and alcohol treatment centers, and area shelters.

MESA Commons

Residential program through the NYS Office of Mental Health.

ABOUT THIS PROGRAM

    • Permanent housing program for individuals living with HIV or AIDS compromised of two- and three-bedroom townhouse units with private entry through a community courtyard.
    • Units are equipped with standard amenities including full bathroom, refrigerator, stove, air conditioning, and a washer and dryer.
    • Leases are required on a month-to-month basis.  Rent includes heat, electricity, hot water, and garbage collection.
    • No security deposit is required.

ELIGIBILITY INFORMATION

    • Individuals 18 years of age or older
    • Documented diagnosis of HIV or AIDS
    • Considered unhoused / homeless or have inadequate housing status and fall below the federal poverty line.

Mental Health

Apartment Treatment Program

Residential program through the NYS Office of Mental Health.

ABOUT THIS PROGRAM

    • Designed for individuals who have thrived in structured environments while their recovery journey and are now prepared to embrace greater flexibility and independence.
    • Residents must adhere to prescribed medication routine, remain abstinent from harmful substance,  maintain a clean living environment and be proficient in basic cooking skills
    • Our goal is to provide a model for healthy living so residents can move into a more independent setting.
    • Apartments are scattered throughout Madison County
    • Program staff are available around the clock, 365 days a year but not on-site

ELIGIBILITY & ENROLLMENT INFORMATION

For enrollment, please contact the Madison County Mental Health Department’s Adult Single Point of Access (SPOA) team at 315-366-2327 or visit them online.

Madison County Mental Health Department’s Adult Single Point of Access (SPOA) department determines eligibility and priority status.  To be considered for the Apartment Treatment Program, applicants must be 18 years of age or older with a mental health diagnosis such as major depressive disorder, bipolar disorder, or schizophrenia.

Venture House

ABOUT THIS PROGRAM

    • Designed for individuals with serious and persistent mental health diagnosis and/or co-occurring substance use disorders 
    • Residents share responsibility for general housekeeping and daily chores
    • Dinner is cooked and served in a family-style manner while residents are responsible for preparing their own breakfast and lunch with staff support
    • Staffed 24/7/365

ELIGIBILITY INFORMATION

    • Individuals 18 years of age and older who are
    • Documented diagnosed with a DSM-V Axis I disorder—classified as a serious and persistent mental illness. To request an application, call the program direct

Substance Use

Maxwell House

ABOUT THIS PROGRAM

    • Designed for individuals in the early stages of recovery
    • On-site life skills development as well as recovery supports and planning
    • Staffed 24/7/365

ELIGIBILITY INFORMATION

    • Adult men 18 years or older with a diagnosis of substance use disorder
    • Meets criteria based on Level of Care determination
    • Maintain abstinence from substance use including alcohol or other drugs

Individuals can apply by completing our Residential Application and submitting to residentialservices@liberty-resources.org as a self-referral.   The agency also accepts referrals from hospitals, case management agencies, correctional facilities, drug and alcohol treatment centers, and area shelters.

Next Step Supportive Living

Residential program through the NYS Office of Addiction Services & Supports.Exterior of Next Step Supportive Living

ABOUT THIS PROGRAM

    • Designed for individuals who are ready for more independence in their substance use recovery journey
    • Apartments and housing are scattered throughout Madison County
    • Program staff are available around the clock, 365 days a year but residents must be able to live without supervision.

ELIGIBILITY INFORMATION

    • Must be 18 years of age or older with a substance use disorder diagnosis.
    • Meets criteria based on Level of Care Determination or have completed a higher level of care
    • Have an established support system and recovery plan
    • Individuals across the board must be able to function without 24-hour-a-day on-site supervision.

Permanent Supportive Housing - Medicaid Re-Design Team & Upstate programs

Residential program through the NYS Office of Addiction Services & Supports.

 ABOUT THIS PROGRAM

    • Designed to assist individuals and families striving for substance use recovery find permanent housing through rental subsidies and care management services
    • Turnkey apartments with the option to take over responsibility of lease once services are no longer needed
    • The agency provides supports through the following programs:  Medicaid Re-Design Team (MRT) and Upstate

 ELIGIBILITY INFORMATION

 MRT Program

    • Adult men 18 years or older with a documented substance use disorder and chronic health or mental health condition
    • Unhoused / homeless or at risk of becoming
    • Enrolled in Medicaid or eligible for Medicaid enrollment
    • Deemed a “high user” of Medicaid services – 5 emergency room visit or 2 inpatient hospitalizations in 12 months prior to application
    • Household income cannot exceed 50% of the median income (see Poverty Guidelines from US Department of Health & Human Services)

Upstate Program

Transitional Safety Units

Residential program through the NYS Office of Addiction Services & Supports.

 ABOUT THIS PROGRAM

    • Designed for adults with substance use disorder who are unhoused / homeless and exiting a residential treatment program or the criminal justice system
    • Provides transitional housing for up to 12 months as the individual pursues permanent housing
    • Includes support for an apartment with the option to take over responsibility of lease once services are no longer needed

 ELIGIBILITY INFORMATION

    • Adults 18 years or older with a documented substance use disorder
    • Unhoused / homeless or at risk of becoming unhoused / homeless
    • Exiting a residential treatment program or the criminal justice system
