Children’s Mental Health HCBS Waiver Program

The Children’s Mental Health HCBS Waiver Program serves children with complex, significant mental health needs in their homes and communities, thereby decreasing the need for placement to intermediate psychiatric inpatient and residential treatment facility levels of care. The goal of the HCBS Waiver program is to give families and youth the supports that are needed in order for the youth to remain at home with their families and in their community. The HCBS Waiver Program serves children ages 5-21 who reside in Onondaga County, along with their families and natural supports. Youth must be enrolled in the program prior to their 18th birthday.

Program History

Liberty Resources became a provider of HCBS Waiver services in 2008. The program serves 18 youth in Onondaga County.

Services

All HCBS Waiver Program Services are individualized and utilize a strength-based approach through the following services: Individualized Care Coordination, Respite Services, Family Support Services, Skill Building Services, Intensive In-Home Services, and Crisis Response Services. Youth and their families enrolled in the program receive visits from their Intensive Care Coordinator 1-2 times per week, skill building services and hourly respite, safety planning, family support, psycho-educational services, a 24 hour crisis line, and ICC collaboration and coordination with therapist, psychiatrist, school and other service providers. All program referrals are made through the Onondaga County ACCESS team.
