OASAS Residential Services: MRT Permanent Supportive Housing
The MRT Permanent Supportive Housing program is overseen by the New York State Office of Alcoholism and Substance Abuse Services (OASAS) and serves individuals in recovery who are currently homeless or are at risk of homelessness and are deemed high users of Medicaid Services. The program is designed to reduce Medicaid cost while providing the necessary support for housing, health and recovery stability, economic self-sufficiency, and community reintegration. Apartments are fully furnished and move in ready. Liberty Resources currently operates eight Permanent Supportive Housing apartments in Madison County. To be eligible for services, individuals must meet the following criteria:
- Must have a documented substance use disorder as well as a chronic medical or mental health condition
- Must be homeless or at risk of homelessness
- Must have Medicaid or be Medicaid eligible
- Must be a high user of Medicaid services or be enrolled in a Health Home Care Coordination program
- Participant/household income cannot exceed 50% of the median income
Supportive services appropriate to the needs of each tenant are provided. This is established through individual assessment and the development of an individualized service plan. Program staff, either directly or through linkages, assists tenants with accessing resources that will enhance their ability to secure employment, including educational opportunities, job readiness skills, vocational training, and employment placement and retention. Apartments are turnkey and all tenants have the option to take over responsibility of the lease where they are residing when it is determined Permanent Supportive Housing services are no longer needed.

3) Include a copy of the person’s Medicaid Card if possible.

A staff member will make contact to schedule a phone screen after receiving application, bio-psychosocial evaluation, income documentation, and a letter confirming homeless status. When admission criteria are met, the applicant will be admitted or placed on a waiting list for services.
