Registered Nurse – Developmental Disabilities
This position offers an amazing quality of life for our potential nurses. The position is Monday through Friday with no on-call. Some of the essential responsibilities include:
- Performing on-site visits and consultations with individuals and/or staff to determine the individual’s current health status
- Providing medical crisis intervention/counseling
- Completing evaluations, medication reviews, and care plans, reviews and progress reports
- Providing education and training to individuals and staff in health-related topics
- Supervising, conducting, and documenting staff medication administration training
- Conducts staff training’s in areas of infection control, CPR and First Aid.
Qualifications: Registered Nurse from an accredited college or school of nursing. One year experience as an RN, preferably working with people who are developmentally disabled. Must possess a valid driver’s license.
Registered Nurse – Integrated Health Care
Liberty Resources Integrated Health Care’s Family Health Center is looking for a Registered Nurse to join the primary care staff (physician, NPs, LPNs, and Medical Assistants) to deliver patient centered care and support clinic operations. Integrated Health Care is a collaborative, multi-disciplinary team of primary care and mental health professionals, including social workers, marriage and family therapists, licensed mental health counselors, psychiatrists/nurse practitioners and registered nurses.
Qualifications: Current NYS Licensure required. Associates Degree in nursing with a minimum of 1 year relevant experience.
Liberty Resources is a seeking a Licensed Practical Nurse to fill a collaborative role within the Health Home Care Management Program and Integrated Health Care Clinic. This collaborative position will work within the Home Health Care Management Program assisting children and their families navigate the healthcare system. The position will also be a resource to the Integrated Health Care Clinic’s multi-disciplinary team of primary care and mental health professionals.
The Licensed Practical Nurse will work with service recipients to determine their immediate and ongoing eligibility for HHCM services and assist with completing the enrollment process. The Licensed Practical Nurse will also provide clinic based direct patient care in collaboration with the registered nurse, medical assistant, peer specialist and patient access representative.
Qualifications: Bachelor’s Degree with two years relevant experience or Master’s Degree with one year relevant experience. Current NYS LPN Licensure.
Per Diem Registered Nurse – Help Restore Hope Center
Liberty Resources, Inc.’s Help Restore Hope Center – HRHC – (formerly known as the Victims of Violence Program) provides comprehensive services to those impacted by sexual assault, domestic violence or other violent. Free and confidential victim services are available to all survivors and their family members. The Sexual Assault Nurse Examiner (SANE) project is a component of the Center, which is a multidisciplinary response that aims to improve post-assault care for survivors of sexual assault, through specialized coordinated services, while at same time increasing assault reporting and prosecution rates. As opposed to hospital emergency department personnel caring for sexual assault survivors alone, the HRHC specially trained SANE nurses will provide comprehensive psychological, medical, and forensic services to survivors of sexual assault in a trauma-informed fashion.
Qualifications: Current Registered Nurse Licensure or higher.
